San Ysidro Health is a non-profit organization established in 1969 and headquartered in San Diego, California. In 2017, San Ysidro Health Center officially changed its name and corporate identity to San Ysidro Health. Additionally, in 2018, San Ysidro Health’s corporate also relocated its office to meet the organization’s administrative needs. The organization offers high-quality, compassionate, accessible, and affordable healthcare and support services for patients of all ages. San Ysidro Health patients enjoy access to comprehensive medical, dental, and behavioral health services, as well as support services and programs that help with other needs like food, housing, and health education. The company has a network of 50 program sites and expanded to over 20 Clinics across San Diego County including various services such as urgent care, pediatric, family, and adult medicine, mobile medical units, women’s health, and more. San Ysidro Health and its program sites are designed under the concept of the Patient-Centered Medical Home (PCMH) model of care, where the patient is the focus of all care and services.
